For larger organizations, Postini seems to be the way to go. We used to use SurfControl, and it just didn't get enough spam, and was a bear to manage, we couldn't find a line between too many false positives and too much spam. We are starting to roll Postini, and it is great, 99% spam capture, very low false positive, and the end users can manage their own settings and speciy things they want to come through. Not cheap, but it is the best product I have seen yet.
